Form 8655 is used to authorize a reporting agent to:

Where To File Form 8962 to IRS

Form 8962 is used to figure the amount of your PTC and reconcile it with any APTC paid.

You can electronically file Form 8962, Premium Tax Credit (PTC), along with your federal income tax return (Form 1040, 1040-SR, or 1040-NR). Filing electronically is the easiest way to file a complete and accurate tax return.

The fax number is on the letter you received from the IRS requesting Form 8962 and Form 1095-A.
